Application
You can apply for this benefit throughout the year and the cut-off date for requests is the 25th of each month; a clear calendar month is required between receipt of your request and the effective benefit start/amendment date (e.g. requests received before 25th November will be valid from 1st January with salary deductions commencing from January's pay).

If you also wish to purchase an assessment for your partner/spouse please complete their details on a MyBenefits online form. You will receive a welcome pack from Nuffield Health and can then book your assessment by contacting them on 0845 230 2040 or via email at centralbookings.wellbeing@nuffieldhealth.com at any time over the next 12 months.

Payment
The cost is spread over 12 monthly instalments deducted from your net pay. You will not need to renew this benefit and instalments will continue unless you cancel your benefit or leave the University.

Cancellation

If you are leaving the University or wish to cancel your benefit you must notify us by submitting a cancellation request via a MyBenefits online form.  If you cancel before having had your annual assessment all monthly payments already made by you will be refunded in your net pay the following month.

If you have already undergone, or booked, your assessment, the outstanding balance of the full annual cost will be deducted from your net pay the following month. If you leave the University and it is not possible to deduct the whole outstanding balance from your final pay, you will be sent an invoice.

Your Assessment

Your assessment is available at any Nuffield hospital, health clinic or fitness and wellbeing centre throughout the country. The nearest locations are Nottingham, Derby, Sheffield and Leicester (please note mammograms can only be conducted at Derby or Leicester Nuffield Hospital). For other locations please refer to the Nuffield Health website http://www.nuffieldhealth.com/health-assessments.

There are three assessment options and information about which tests are conducted within each option is contained within the Assessment Options, and additional information within the FAQs document. After your first assessment Nuffield Health will contact you annually to arrange your next assessment.
